Who we are and how to contact us

Local Foot Care is responsible for the personal information it uses to manage enquiries and provide its services. Contact Andrew at Local Foot Care, Hatch Ride, Crowthorne, RG45 6LB, on 07932 630 969 or through our website Contact form. We do not provide an email address for enquiries.

Information we collect

We receive the information you give us by phone, through our forms and when arranging or receiving care. This may include your name, contact details, visit address or postcode, enquiry, appointment details, relevant health information and treatment records. Referral forms may also contain the referred person’s name and your relationship to them. Please only share another person’s details with their permission or appropriate authority. Avoid putting detailed medical information in a general website enquiry.

Why we use information

We use information to respond to enquiries, arrange appointments, provide appropriate care, keep treatment records, administer payments and deal with concerns. For enquiries about your own appointment and delivery of agreed services, the legal basis is taking steps at your request before entering a contract or performing that contract. For general enquiries, referrals and handling concerns, we rely on our legitimate interests in communicating with people and managing our service, balanced against their rights. Where records are required by law, we rely on the relevant legal obligation.

Health information

Health information receives additional protection. Information necessary to provide care is handled confidentially for the provision of health care or treatment, subject to the applicable duty of confidentiality and safeguards. This is separate from your consent to treatment. If we need explicit consent for another use of health information, we will ask for it separately and explain how it can be withdrawn. Please contact us if you want an explanation of how a particular record is used.

WriteUpp, Wix and sharing

Patient records are stored in WriteUpp. Our website and its enquiry forms use Wix. These providers process information as needed to supply their services. Access and disclosure should be limited to what is necessary for the purpose. Information may also be disclosed where legally required or where necessary to establish, exercise or defend legal claims. We will discuss care-related sharing with you where appropriate. WriteUpp states that its UK and European service is hosted in the EU. Service providers may process information outside the UK; applicable adequacy arrangements or other lawful safeguards are required for restricted transfers. You can ask us for further information about relevant recipients and safeguards.

How long we keep records

Patient records are stored in WriteUpp and retained for as long as required by applicable legal obligations, taking account of relevant professional guidance and insurance requirements. Records are reviewed and securely deleted when no longer needed. Retention decisions take account of the record type, the date care ended, the patient’s age, and any outstanding complaint or legal claim. Enquiry information is kept only while needed to deal with the enquiry and related follow-up or record-keeping obligations. Please contact us to ask about the retention period applying to your records.

Your rights

Depending on the circumstances, you can ask to access or correct your information, request erasure or restriction, object to its use, or receive certain information in a portable form. These rights have conditions and exceptions; a request to delete records may not override a legal retention requirement. If a use relies on consent, you can withdraw that consent without affecting earlier lawful use. Contact us using the details above. We may need to confirm your identity. You can complain to the Information Commissioner’s Office at ico.org.uk/make-a-complaint/.

Cookies and external websites

This Wix website uses cookies and similar technologies to support its operation. Your browser settings can control or delete cookies, although blocking them may affect site functions. Where the site provides cookie preference controls, you can use those controls to manage your choices. This notice does not itself give consent to optional cookies or marketing. Links to 21st Century Clinic and other external services take you to separately operated websites; please read their privacy notices before submitting information there.

Updates

We may update this notice when our services or information-handling arrangements change. The date above identifies this version.

A legal disclaimer

The explanations and information provided on this page are only general and high-level explanations and information on how to write your own document of a Privacy Policy. You should not rely on this article as legal advice or as recommendations regarding what you should actually do, because we cannot know in advance what are the specific privacy policies you wish to establish between your business and your customers and visitors. We recommend that you seek legal advice to help you understand and to assist you in the creation of your own Privacy Policy.

Privacy Policy - the basics

Having said that, a privacy policy is a statement that discloses some or all of the ways a website collects, uses, discloses, processes, and manages the data of its visitors and customers. It usually also includes a statement regarding the website’s commitment to protecting its visitors’ or customers’ privacy, and an explanation about the different mechanisms the website is implementing in order to protect privacy.

Different jurisdictions have different legal obligations of what must be included in a Privacy Policy. You are responsible to make sure you are following the relevant legislation to your activities and location.

What to include in the Privacy Policy

Generally speaking, a Privacy Policy often addresses these types of issues: the types of information the website is collecting and the manner in which it collects the data; an explanation about why is the website collecting these types of information; what are the website’s practices on sharing the information with third parties; ways in which your visitors and customers can exercise their rights according to the relevant privacy legislation; the specific practices regarding minors’ data collection; and much, much more.
